Show custom fields in the UI as if they were regular fields

At the moment custom fields live under an expandable UI. As well as causing UX issues, this doesn't seem to be necessary.

Describe the solution you'd like.

Display custom fields as if they were just regular fields, remove optional labels.

Describe alternatives you've considered

Default custom fields to expanded, fix alignment and remove placeholder.
